  This form is to be used for those who do not want to pay by credit card nor electronic check.  To use this form you must first turn on your printer then fill out all the information.  When you click on submit (to submit your order) your printer window will open.  Set your printer to make 2 copies and set to print Page 3 only.  DO NOT PRINT ALL.  Then send ONE copy of it with your money order to:    HexaDyne LLC,  Atten: Orders,  8635 W.
